2. In field «username» write your login
3. In field «password» write your secret key (your password must use at least one number)

If you forgot your username: put on link under field «Username» - forgot your username? In new page –fill in information fields:

• Street address
• ZIP code
• Date of birth
• Gender

Registration with Prescription Number

For first step - your need enter information in column into registration form on the site. There is:

Put your Prescription number in first column – its 9-digit mail order Rx number. Enter your date of birth. Put the button «Continue».

Registration with Member ID

In first line your need enter your Member ID. Remember, if your Member ID ends 01,02,03,04 … Enter him without this number. Enter information form:

• First Name
• Last Name
• Date of birth

Put button «Continue».

Registration without Member ID (No have)

In this case, enter all fields in information form:

1. First name
2. Last name
3. Date of birth
4. Street address
5. ZIP code
6. Gender (Female or Male)

Put button «Continue».
